As a participant in Chester County Futures, it is my intent to prepare myself for an education. I realize that if I do not follow through with these responsibilities I can be removed from the program. As an individual pursuing a post-secondary education, I will do the following:

• I will maintain a “C” or better in all of my classes throughout the school year. If my grades fall below a “C”, I will ask for help from my teachers and my program coordinator.

• I will complete all school assignments on time.

• I will make school attendance a priority by attending school regularly, arriving on time and having no unexcused absences.

• I understand that Chester County Futures meetings are MANDATORY, (this includes afterschool and morning meetings) and I will attend. If I am unable to attend, I will contact my program coordinator in advance.

• I will be involved in extracurricular and community activities, including participating in Chester County Futures community service projects.

• I will contact my program coordinator at least once a month.

• If I am assigned a one-to-one mentor, I will meet monthly with my mentor and talk to him/her by phone or e-mail at least once a week.

• I will read all Chester County Futures information that is sent to me and share this information with my family. I will respond to invitations by the response date.
